AHMR (Annual Health and Medical Record)

This is required for every participant, including scouts, parents and siblings!  The forms are good for all Pack and Den activities for the year and need to be updated annually.

Family Talent Survey

Help share your time and talents with the Pack!  This information gathering survey offers a plethora of ideas for finding your place in scouting.  We are completely volunteer-run and the pack depends on the enthusiasm of yo-yo hobbyists, closet paleontologists, and any parent with a love for fun!

Photo Release

As a participant in Pack 100 Westborough Cub Scouts, you hereby grant Pack 100 full rights and permission to publish on the Pack 100 website, social media, and recruiting materials, any or all photographs or electronic representations taken of any of me or of my family members at any scouting activity or event. No names, phone numbers, or addresses will be published with photos. If you choose to revoke this permission, please complete and return this form to the Pack Committee Chair and your family members will be removed from photos.
